Try this: ICD-9-CM Guidelines were created on 08/31/2009 and are effective as of 10/01/2009. An ICD-9-CM Code Conversion Table table for diagnostic and procedural code changes between 1986-2009 is provided to assist users in data retrieval. The table shows the date the new code became effective and its previously assigned code equivalent.

I am a student at DeVry University. My major is Health Information Technoloy. I am finishing a presentation on Medicare Inpatient Diagnosis in the state of Illinois. I am glad to have come across this website for updates of the ICD-9 for the year 2010. I found the this website to be very informative and I am using it as a reference in my presentation.

Significant, Separately Identifiable Evaluation and Management Service by the Same Physician on the Same Day of the Procedure or Other Service: may be necessary to indicate that on the day a procedure or service identified by CPT code was performed, the patient’s condition required a significant, separately identifiable E/M service above and beyond the other service provided or beyond the usual preoperative and postoperative care associated with the procedure that was performed.
